Elderly Leaving Hospital To Be Assigned Doctor Or Nurse
A named clinician will be given responsibility for the care of an elderly person discharged from hospital, in an effort to create a "more personal service".
The scheme, to be announced by the health secretary during an event marking the 65th anniversary of the NHS, will see elderly people leaving hospital be assigned a clinician who is then responsible for their care.
Some critics however have said that the new scheme is being used to overshadow real issues with the National Health Service.
Mr Hunt is expected to say: "The challenge today is to provide integrated, co-ordinated, out of hospital care. Something where the NHS, with our tradition of family doctors and primary care, could lead the world."

The survey of 466 dentistry practices - which comes a year after the start of new NHS dentists' contract in England - found that just over a third (36%) are taking on any new NHS patients.

Reid gives 'green light' for £4bn PFI hospital deal
The 'green light' has been given for 15 new NHS hospital developments in a Private Finance Initiative (PFI) deal worth more than £4 billion, it has been announced today. Today's announcement will bring investment in new NHS hospitals to more than £16 billion since 1997, the health department said.
