Death Inquiry Data 'Missing In Post'

The Ministry of Justice (MoJ) has confirmed that discs, containing information from three sensitive death inquiries have gone missing – after they were put in the post.

Officials first discovered the two discs were missing on 8 January and steps were immediately taken to locate the items. They have not yet been located.

A member of staff has since been suspended.

In a statement, the MoJ said the missing material relates to inquiries into the role of the police in the deaths of three members of the public. Two of the affected cases relate to fatal police shootings of crime suspects in London; Mark Duggan and Azelle Rodney. The third investigation is in relation to the murder of Robert Hamill in Northern Ireland in 1997.

Mr Hamill, 25, was beaten by a loyalist mob in Portadown, Co Armagh, in April 1997. The father-of-three died 11 days after the attack and it has been alleged by campaigners that his death involved the collusion of police officers.

The Ministry of Justice has said it views the loss "extremely seriously", and there is no evidence to suggest the loss was the result of "malicious intent".

The families of the three men have been informed of the situation.
